POSITION SUMMARY:
The Health & Wellness Specialist provides professional nursing oversight, training, and
involvement when needed across the portfolio of Pegasus Senior Living Communities. The key
roles of the Health & Wellness Specialist include: Nursing clinical oversight, Nursing delegation
where needed, clinical system support throughout the MatrixCare Conversion, temporary
assignment to a Community as Health and Wellness Director and assist with Clinical Training
programs.


ESSENTIAL DUTIES:
1. Always Strive to make EVERYDAY the BEST DAY EVER by modeling the tenets of the

2. Adheres to all policies and procedures by the State Board of Nursing and State Regulatory
agency.
3. Delegates specific tasks to personal care staff, such as all new medications and treatments
that require a Nurse delegator’s oversight
4. Responsible for assessing all residents when they undergo condition changes documents
assessments and care plans and ensures they are signed and documented into Matrixcare if
required.
5. Responds to resident changes in condition with timely assessments to ensure that resident
care needs are met through assessment, planning, interventions and follow up as outlined in
the service planned.
6. Performs a thorough health assessment on all residents within 30 days of move-in to the
community or per State-specific regulations also monitors the health status of current
residents through periodic assessments and as required.
7. Monitors staff compliance with all local, state, federal and licensing rules and regulations
associated with the licensure of the community as they pertain to clinical and risk
management activities. This includes HIPAA, OHSA, and State Survey issues, etc. that
apply to the specific location of the property.
8. Participates in resident care conferences if needed.
9. Assist in covering on call duties, respond to clinical issues, and report issues to the Health &
Wellness Director in a timely manner to ensure an appropriate response and follow-up with
care issues.
10. Responsible for advising all care staff of the appropriate steps to take in caring for residents,
work in a safe manner, and report unsafe actions to the Health & Wellness Director or
Executive Director.
11. Responsible for notifying the appropriate management team member if workplace injuries
occur, advise the staff of the injury and following specified procedures of documentation and
medical assistance.
12. Responsible for fostering teamwork with co-workers, showing initiative in overcoming
obstacles, takes actions to assist others beyond job responsibilities, and has a positive impact
on co-workers, residents and visitors.
13. Work closely with the Pegasus Clinical Leadership Team to identify opportunities and
obstacles with each conversion project and actively problem solve issues.
14. Takes direction form the Regional Health & Wellness Director and or VP of Health &
Wellness on the specific duties related to each MatrixCare conversion project.
15. Acts as the Health & Wellness Director in communities that have a vacancy and are
considered a focus community.

4. Ability to communicate and work within all levels of the company while following reporting

5. Ability to understand the functionality of MatrixCare and trains and support the staff.
6. Ability to relate to the associates, public, residents, family members and other professionals
effectively.
7. Ability to teach/train others effectively.
8. Ability to work flexible hours to meet requirements of the job.
9. Ability to travel up to 50%.
10. Ability to safely operate a motor vehicle and must possess a valid current driver’s license and
current auto insurance.
11. Ability to maintain accurate records and provide information as requested/required.

MINIMUM JOB QUALIFICATIONS:
Bachelor’s Degree in Nursing, or related field
Licensed Nurse with State specific accreditation or license required
3 years+ management experience in assisted living, Alzheimer’s or Memory Loss care.
Exceptional multi-tasking skills
Strong problem-solving skills
Experience in Residential Care Management, Clinical Director, or Risk Management
