Job Description
The Data Analyst, Social Determinants of Health & Social Services is responsible for assisting with the development and management of data reporting systems for programs and initiatives related to the Social Determinants of Health and Social Services. Duties include but are not limited to developing and maintaining SharePoint sites for social service related programs, managing data extraction and reporting from third-part case management programs, and creating and managing data reporting structure for initiatives with external community partners.
Essential Functions
- Develop in-depth understanding of third-party case management system and develop data extraction guidelines
- Develop, plan, and maintain SDH Program SharePoint Site, including the building of sub-sites, granting user permissions, and site graphics.
- Works with program leadership and BI department to establish key program metrics, generate monthly reports and acquire understanding of program/initiative objectives.
- Create productivity reports for agency leadership and funding sources
- In conjunction with program leadership, liaise with external community partners to develop data tracking and reporting mechanisms. Current programs include First LINK, Food Rx, and 5th Ward Go Neighborhood initiatives. Additional projects to be added.
- Recruit, train, and support student intern or AmeriCorps member in managing patient appointments for First Link program.
- Build capacity of First Link program for expansion to additional sites. Direct corrective action as needed to maintain the viability of the program.
- Other duties and special projects as assigned or requested.
- Maintain strict confidentiality per Legacy and HIPPA guidelines.
- Participate in Legacy’s Performance Improvement Program.
- Ability to travel between sites as needed.
Education & Training Requirements
- Bachelor’s degree required in Public Health, Information Systems or related degree from an accredited institution
- Masters’ degree preferred
- All licensed professionals must maintain their professional licenses and comply with all applicable recognitions/designations as defined by the respective Texas licensing/examining boards.
Work Experience
- Must have a minimum of 2 years’ experience in social services and or health related setting.
- Must possess excellent interpersonal and communication skills, both verbal and written.
- Good knowledge of Microsoft Office suite, SharePoint, and ability to rapidly learn additional software systems as needed.
- Strong track-record delivering timely data reports in an easily accessible format to be utilized by funders and program leadership
- Strong track-record of innovation and initiative in creating data systems
