Position Summary:

The Memory Care Program Coordinator coordinates all aspects of life for Memory Care residents including evaluating residents upon admission and developing a personalized plan or program for successful aging in the community.  This person will ensure resident satisfaction and motivate residents to function at their highest level of independence. 

Responsibilities:

  • Oversees and ensures completing all admission and evaluation materials ensuring appropriate documentation including functional assessments.
  • Ensures that Memory Support programming is occurring and appropriate for residents.
  • Motivates residents to function independently.
  • Ensures that all Resident Rights and Responsibilities are protected and enforced.
  • Ensures compliance of all State Rules and Regulations of Memory Care.
  • Establishes and maintains a close liaison with residents and families.
  • Maintains all records and communications as required by state agencies.
  • Ensures compliance with all Fire Safety Procedures including the scheduling and execution of mandatory fire drills.
  • Responsible for the transfer/discharge process whereby residents are transitioned smoothly to higher levels of care, as required. Counsels with resident representatives as needed.
  • All other duties as assigned and supports resident culture of Successful Aging.

Qualifications:

  • Minimum of one-year experience, preferably in a hospital or nursing home.
  • Associates degree or equivalent experience.
  • Previous supervisory experience required.
  • Obtain a TFER (Texas Food Establishment Rules) food handler certification within 60 days upon hire.
  • Activity Director Certification.
