Responsible for general operations of memory care neighborhood including program development, resource management, scheduling, care plan coordination, and supervision of resident care.
A few other details about the role:
Programming
- Develop cutting edge, best practice dementia programming utilizing available resources and incorporate person-directed care.
- Coordinate with lifestyle lead to create a monthly Lifestyle calendar that is specific for residents living in the memory care neighborhood.
- Create at least 12 hours of ability-centered care programming daily that is posted for residents and families.
- Follow the “Life Reflections Memory Care Program Philosophy and Programming Guidelines”.
- Host quarterly family nights for residents in the memory care neighborhood with the administrator.
Resource
- Act as a role model, continually demonstrate to all team members, residents and family that the community is committed to the creation of the pleasures and good daily life expected of home and person-directed care.
- Responsible for ongoing Alzheimer training of memory care team.
- Conduct family educational sessions on dementia, quarterly or as needed.
- Participate in general orientation of health center team members and other disciplines. • Develop a library/resource center providing information on dementia
- Conduct daily stand-up meetings.
- Provide dementia-specific training annually and for new hires according to regulatory requirements. Conduct educational presentations for internal and external community.
Marketing
- Coordinate resident admissions to the neighborhood, including both resident and family.
- Working with nursing, conducts assessments of prospective residents for their capabilities and needs. Assist with developing a person-directed service plan.
- Assist in meeting or exceeding budgeted census for the memory care neighborhood • Coordinate endowed admissions to the health center.
- Develop relationships with other providers and professional resources.
- Maintain inquiry log for memory care.
- Attend weekly admissions/sales meetings.
- Participate in branding product/services offered in memory care.
- Complete a minimum of 4 outreach activities monthly.
Management
- Make daily rounds, assessing resident’s cognitive status and team participation to insure that the resident’s individual needs are being met and that the resident is an active part in how to meet these daily needs.
- Make daily rounds to inspect environment and equipment for repair or maintenance needs and cleanliness, reporting findings to the appropriate party
- Participate in weekly care plan meetings with interdisciplinary team and family/resident care plan conferences.
- Prepare and maintain pertinent and appropriate records, reports, studies, and schedules.
- Participate in quality assurance for memory care and nursing quality assurance for the health center.
- Allocate and monitor resource utilization for memory care in conjunction with the Administrator.
- Responsible for on-call duties for health center as assigned on rotational basis.
- Participate in all meetings, in-services, seminars, and committees, as designated.
- Attend educational sessions to maintain expertise in care of residents with dementia.
- Manage departure alert and wander management system which includes maintaining daily log that system is working, weekly checks of resident alert appliance, maintaining an adequate supply of appliances, change, add, update and delete appliances as necessary in the departure alert system.
- Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
- Obtain knowledge and demonstrate the principles of Living Lifespace , person directed care and successful aging philosophies and their inherent core values of commitment, respect, compassion, dedication, team work and quality. Team member will support, actively participate and act in accordance with the principles of the Living Lifespace service culture and be familiar with and fluent in the language and service pillars of Living Lifespace.
- Maintain regular and punctual attendance at work and meetings.
Administrative
- Meet weekly with administrator.
- Develop policies and procedures in compliance with State and Federal regulations and the Guidelines for Dignity established by the Alzheimer’s Association as needed.
- Oversee documentation to ensure requirements are met per neighborhood regulations. Complete social service required documentation, M.D.S. and care plans. Communicate effectively and timely with responsible party regarding any changes in resident’s status or resident concerns.
And here’s what you need to apply:
- Associate’s degree in related field advanced training/knowledge regarding care for residents with Alzheimer’s disease or other dementias. Bachelor’s degree preferred.
- Two-three years applicable experience.
- Or equivalent combination of education and experience.
Certifications and licensure required by state regulations.
- Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, vendors and team members.
- Computer experience needed.
- Actively enforces resident and employee confidentiality and rights.
