Position Summary:
The Speech-Language Pathologist provides professional support services which may include evaluation and treatment of individuals with speech, language, hearing, oral motor, or swallowing disorders.
Job Responsibilities:
- Complete the initial evaluation and admission for individuals admitted for Speech therapy services and develop a plan of care.
- Provides treatment for individuals to improve speech, language, hearing, oral motor, or swallowing disorders.
- Observes, records, and reports to the attending physician and other staff, the individual's reaction to treatment and any changes in the individual's condition or plan of care.
- Instructs the individual and their caregiver when applicable in the care and proper use of equipment and devices. Also, advises and consults with physicians regarding the feasibility of equipment and devices. Instructs other planning team personnel, family, and/or caregivers in assisting with the implementation of the Individual Support Plan when applicable.
- Evaluate the individual's progress monthly and submit a monthly progress report.
- Document appropriate progress and clinical notes indicating the individual's response to therapy.
- Schedules and conducts treatments and consolation according to individual's response to therapy.
- Coordinates discharge planning as appropriate.
- Confers with other disciplines as needed.
- Documents time, date, and daily visits per company policy.
- Completes and submits required documentation within 24 hours of visit.
- Maintains positive relationships with individuals, co-workers, staff, physicians, and others.
- Position in non-exempt.
- Maintains established agency policies and procedures, objectives, safety, environmental, and infection control policies as well as maintaining confidentiality and HIPPA regulations. Maintain MDC policies and handbook.
- Maintain a valid driver's license.
- Gain knowledge of the Individual Support Plan (ISP), Training Specifics to the Needs of the Individual (TSNI), Behavior Support Plan (BSP), individual programs, and therapy plans of the assigned service recipient caseload. Follow all programs daily and report progress on outcomes and actions. Participate in ISP, Circle of Support (COS), and staff meetings to give input on the effectiveness of programs for your service recipients.
- Attend all scheduled, required training and updates and complete all training successfully and promptly as per IDID requirements. Training will be provided in the classroom as well as electronically through Relias.
Minimum Education and Experience:
- Masters level degree or higher in Speech-Language Pathology.
- Current Tennessee licensure as a Speech-Language Pathologist.
Knowledge and Abilities:
- Perform physical aspects listed in the job description
- Must possess moderate computer skills, including the use of Microsoft Office, the ability to learn the Timas database, and the EMR program.
- Must be able to communicate professionally in English both orally and in written form.
- Caring for people with a desire to advocate for people supported at MDC.
