Purpose:
The Registered Nurse (RN) provides services and/or treatments requiring substantial and specialized nursing skill and provides effective and efficient patient care in accordance with applicable law and regulations, accepted standards of nursing practice, payor requirements and Interim HealthCare protocols and procedures.
Essential Duties:
- Participates in interdisciplinary patient assessments to evaluate needs at appropriate times, contributing to comprehensive care planning.
- Collaborates in the development and evaluation of the care plan, working with the patient, their representative (if applicable), and caregivers.
- Clarifies and incorporates patient goals into the plan of care/service plan to ensure alignment with the patient’s needs and preferences.
- Provides physician-ordered services as outlined in the care plan, ensuring all clinical directives are met.
- Counsels and educates patients and families to address healthcare needs, ensuring informed decision-making and self-care.
- Initiates preventative and rehabilitative nursing procedures to promote optimal health outcomes.
- Completes timely, accurate clinical documentation, tracking patient progress and updating care plans as needed.
- Coordinates referrals to other services, ensuring patients receive comprehensive care in line with their needs.
- Monitors patient care outcomes, evaluating progress and adjusting interventions as required to meet care goals.
- Communicates effectively with the healthcare team, ensuring seamless coordination of care, discharge planning, and updates to the patient’s care plan.
Required Qualification:
- Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the state(s) in which he/she practices required.
- Must have a Valid CPR certification.
- Must have a Valid HIV/AIDS course.
- Must have at least 1200 hours of experience as an RN or completion of the Office's preceptor program required.
- Must provide proof of a Valid Driver's License.
- Must provide proof of Valid auto insurance.
- Must Pass a Drug Screening.
Preferred Qualifications:
- Must have at least 1600 hours of experience as an RN or completion of the Office's preceptor program required.
Working Conditions:
- Able to constantly travel within geographic area serviced by office from assignment to assignment.
- Meets the applicable health requirements to provide patient care per applicable law or regulation.
- Work is normally performed in patient’s home, schools, or occupational settings.
- Frequent exposure to communicable diseases, bloodborne pathogens and/or other potentially infectious or hazardous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
- Able to constantly stand, walk, bend, stoop, squat, kneel and reach freely.
- Able to frequently lift, reposition, and transfer patients.
- Able to constantly grasp with thumb in opposition to fingers of palm, i.e. manipulate pen, knobs or objects.
- Medium physical activity that may require occasional lifting, pushing or pulling up to 50 lbs. in order to carry out daily job functions and related activities that may be required.
