The Quality Assurance Case Manager is responsible for monitoring the overall health and well-being of the person supported.

Job Responsibilities:
1. Ensure the database has updated insurance cards and numbers loaded correctly for assigned caseload
2. Must check communication notes monthly and email to director/assistant director for assigned case load
3. Complete file reviews and notify director/assistant director when documentation is incomplete or missing for assigned caseload
4. Schedule, attend, and notify all departments of Annual ISP/PCSP, COS meetings and High Risk Reviews. Record meeting minutes for each.
5. Attend bi-weekly Event Management meetings
6. Complete all Risk Assessments (update annually and as needed) for assigned caseload
7. Sign in, Scan and Save all MARs into Timas, then file.
8. Educate individuals on their rights (Rights Assessment and Rights Policy)
9. Completion of annual documents
10. Gather conservatorship/Court documents (as needed for new or changed)
11. Review, Scan and Save fall screening tools
12. Add/update contact information into the Timas database for Person Supported as needed
13. Create (with COS) and update HRC fading plans
14. Responsible for activating/deactivating outcomes/actions steps and ISP/PCSP’s in the Timas system
15. Attend training as scheduled by the Director of Quality Assurance
16. Assist with audits as needed
17. Advocate for the Person Supported and the Michael Dunn Center
18. Reviewing, saving Section C’s under cost plans and notifying department if a service is ending or denied.
19. Debt Agreements when needed
20. Scan/Rename and Save all documentation into Timas database after review for accuracy
21. Add/edit contact information into the Timas database for Person Supported as needed
22. Maintain a professional attitude and relationship with individuals and their families, co-workers and personnel from other agencies and businesses.
23. Liaison for Independent/Support Coordinators
24. Adhere to all agency Policies and Procedures and Employee Expectations
25. Complete all training in a timely manner.
26. General work hours are Monday through Friday 8am-4pm (this is flexible based on supervisor approval)
27. Any and All overtime must be approved in advance
28. This position is non-exempt
29. Required to be in office 2-3 days per week

· Employees working remotely must participate in any scheduled virtual meetings, on-camera, with minimal distraction and while maintaining appropriate confidentiality.
· Failure to follow policy, rules and procedures may result in termination of the flexible work arrangement and/or disciplinary action up to and including termination
· Employees must maintain a professionalism even when working remotely.
· Employees working remotely may not allow non-work-related events, activities, persons (e.g., a spouse, partner, children, or non-work-related visitors) or pets or other animals who share the remote work location to disrupt or interfere with their workday or job performance. The responsibility to focus on and productively accomplish your tasks is the same whether working at the office assigned work location or when working remotely.
· Employees working remotely may be required to attend in-person meetings, trainings, or events at the office or other locations as directed by their supervisor(s) or as necessary for the performance of their job duties.
· Employees shall not perform personal business during hours agreed upon as work hours.
· Absences may be an approved flex workday schedule or compressed workweek, employees are required to work normal business hours.
