Position Overview:

As an RN on our Supports Coordinator Team, you will play a crucial role in assessing and coordinating the care needs of our participants. Your work will directly impact their ability to live independently and with dignity through tailored, person-centered plans.

Key Responsibilities:

  • Conduct Comprehensive Assessments: Evaluate participants’ social, financial, environmental, health, and emotional needs to create well-rounded care plans.
  • Develop Person-Centered Plans: Collaborate with participants and their support networks to design and implement effective service plans.
  • Advocate and Coordinate Care: Assist with and advocate for the participants in receiving the care they need according to their personalized plans.
  • Maintain Accurate Records: Manage electronic files and track participant progress, ensuring compliance with state standards and agency procedures.
  • Connect with Community Resources: Provide valuable information and assistance in linking participants to community services, support and providers that are available.
  • Timely and Complete Documentation: Keep precise records of assessments, care plans, and progress notes, adhering to agency and MDHHS guidelines and established timeframes.
