Position Overview
Career Links is seeking a compassionate and motivated Occupational Therapist (OT) to join a collaborative rehabilitation team within a Skilled Nursing Facility (SNF) setting in Minnesota. In this role, you will evaluate residents, develop individualized treatment plans, and provide skilled occupational therapy services that promote independence, safety, and improved quality of life.
This position is ideal for clinicians who enjoy working with the geriatric population and value a team-oriented approach to patient-centered care.
Responsibilities
- Evaluate residents and develop individualized treatment plans based on clinical findings and functional goals.
- Provide skilled occupational therapy services to improve activities of daily living (ADLs), mobility, strength, coordination, cognition, and overall functional independence.
- Establish measurable goals and modify treatment plans as needed based on patient progress.
- Complete accurate and timely documentation in accordance with facility, state, federal, and payer requirements.
- Collaborate with Physical Therapists, Occupational Therapy Assistants, Speech-Language Pathologists, nursing staff, physicians, residents, and family members.
- Participate in interdisciplinary care conferences and discharge planning.
- Educate residents, caregivers, and staff on therapeutic techniques, adaptive equipment, and safety strategies.
- Maintain compliance with all professional, ethical, and regulatory standards.
Qualifications
- Graduate of an Accreditation Council for Occupational Therapy Education (ACOTE)-accredited Occupational Therapy program.
- Current Minnesota Occupational Therapist License issued by the Minnesota Board of Occupational Therapy Practice, or eligibility to obtain licensure prior to start date.
- Current NBCOT certification or eligibility as required for Minnesota licensure.
- Skilled Nursing Facility, rehabilitation, or geriatric experience preferred but not required.
- Strong communication, documentation, and organizational skills.
- Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
