POSITION GOAL

 

The 1st Five Site Coordinator oversees the 1st Five Healthy Mental Development Initiative including all services, operations and compliance of the 1st Five Program for the Family & Community Health Alliance.  This position will be primarily responsible for establishing and maintaining strong relationships with local primary care medical practices.  This relationship includes regular communication on 1st Five referral status and program maintenance, as well as, training and consultation for medical sites.  In addition, 1st Five Site Coordinators are the face of 1st Five in their communities and serve in the capacity as messengers about the importance of young children’s healthy mental development to community stakeholders.

 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

 

  1. Promote the use of developmental surveillance and screening for social emotional-development, family stress, parental depression, and autism, by local primary care medical providers.
  2. Establish and maintain strong relationships with medical provider sites including regular communication regarding referral status and program maintenance.
  3. Responsible for administrative and supervisory tasks for the 1st Five Program including interpreting state and federal guidelines and working with the FCHA Director and Finance staff to develop the 1st Five budget.
  4. Maintain ongoing relationships with county boards of health in the performance of core public health functions of assessment, assurance, and policy development for the maternal and child population.
  5. Responsible for the completion and oversight of time studies related to 1st Five program activities.
  6. Responsible for oversight of site visits and attending required state meetings.
  7. Responsible for the compilation of program statistics.
  8. Responsible for completion of and oversight of all care coordination for 1st Five families.
  9. Responsible for meeting all IDPH requirements.
  10. Participate in public relations activities and community task forces as assigned by FCHA Director.
  11. Recommend operational policies/procedures which meet all standards and regulations in conjunction with the FCHA Director.
  12. Assist the FCHA Director in the preparation of annual grant applications and year-end progress reports.
  13. Receive and act upon technical assistance from the Iowa Department of Public Health, Maternal Health and Child Health consultants.
  14. Provide leadership, direction, supervision, and support to 1st Five Developmental Support Specialists and delegate to others as appropriate.
  15. Assure compliance within the 1st Five Program.

QUALIFICATIONS

Education and/or Experience

     

  1. Must be a health professional with a bachelor’s degree in health education, counseling, nursing, sociology, or psychology OR a graduate of a Bachelor’s degree program or higher in social work.
  2. Must possess basic knowledge of child health development and related community services.
  3. Must complete 1st Five staff training requirements within 6 months of hire.

 

 

Skills and/or Abilities

 

  1. Ability to communicate well and work effectively with a variety of socio-economic groups
  2. Excellent organizational skills
  3. Adept at giving presentations and comfortable speaking to groups of people
  4. Ability to work with little supervision

CONDITIONS OF EMPLOYMENT

 

  1. Complete Child Abuse Reporter training within six (6) months of employment and update every five (5) years.
  2. Complete bloodborne pathogen training within 1 year of employment and update every year.
  3. Receive clearance by the Iowa Department of Human Services on child abuse and criminal records checks upon employment and resubmit/receive clearance every two (2) years.
  4. Valid driver’s license
  5. Valid car insurance
  6. Reliable transportation for on-the-job use
  7. Maintain required continuing education hours to retain state licensure
