Mennonite Village is a well-respected CCRC in Oregon's beautiful Mid-Willamette Valley that provides life-enriching services to people of all faiths and beliefs. We are currently seeking a Full Time Resident Care Manager-RN for the Mennonite Home, skilled nursing facility. The facility cares for post-acute care patients, hospice patients, and residents who reside at the facility. Residents admit with a variety of skilled nursing needs including pain management, management of peripheral IVs, management of central lines and infusions, wound and skin assessment and care including management of wound vacs, administration of medications including injectable. Primary Days/Hours will be Monday-Friday Day Shift.

ESSENTIAL FUNCTIONS:

  • Manages the nursing care of her/his assigned residents. Works with interdisciplinary team to provide most effective delivery of care within fiscal and insurance guidelines.
  • Assures MDS is completed and singed by due date.
  • Coordinates the nursing function and tasks for residents with physicians and other health care providers.
  • Develops and documents nursing plan and care plan within federal guidelines and signs the care plan. Evaluates the effectiveness of services provided to residents.
  • Performs and accurately documents the steps in nursing process and updates documentation as indicated by changes in resident condition in accordance with federal and state regulations.
  • Ensures that all necessary admission assessments are carried out by self or other staff within federal guidelines. Monitors completion and accurate documentation of resident’s capabilities and needs within federal guidelines.
  • Delegates to other licensed personnel and nursing assistants only those functions and tasks for which they are competent and qualified to perform under ORS Chapter 678.
  • Responsible for ensuring that necessary admission assessments are done such as:
  • Resident admission assessment within 8 hours and restorative assessment within 24 hours
  • Bedside care plan placed in assigned area.
  • Preliminary care plan within 24 hours of admission.
  • Rehabilitation plan is implemented within 24 hours of admission and reviewed and updated as frequently as resident’s condition changes, and no less than quarterly.
  • Supervises staff including training, assigning work, appraising performance, coaching, and administering discipline, which may lead to termination.
  • Protects and Promotes residents’ rights as identified in the “Mennonite Village Residents’ Rights”.
  • Keeps current on resident’s health care needs through communication with Charge Nurse. Gives direct care to critical residents or when indicated by staffing requirements.
  • Completes documentation responsibilities accurately, including medication sheets, treatment sheets, progress notes, physician’s orders, occurrence reports, wound and skin stamp, infection control reports, lab results, X-ray results, gathering data for care plan, and informing MD of changes in resident’s condition.
  • Ensures that Mennonite Home Pain Policy/Procedures are met.
  • Takes measures to resolve and prevent negative outcomes such as decubitus ulcers and other skin breakdowns, loss of mobility, development of contractures, or foot drop.
  • Focuses efforts on the restorative needs of residents such as mobility, self-care, and bowel and bladder program.
  • Follows unit policy regarding physical and chemical restraints.
    • Ensures resident’s right to be non-restrained.
    • Monitors and evaluates the use of psychotropic medications with the goal of eliminating unnecessary drugs through monthly PAM meetings.
  • Upon expected discharge of resident, prepares discharge summary prior to, or at discharge.
    • Ability to safely operate a Hoyer lift in accordance with the safe handling (zero lift) policy and procedure.
  • Use and model safety measures for residents and self, with assistance as needed, according to the safe handling policy by using gait belts, proper body mechanics, Hoyer lift, etc. as ordered.
  • Promotes culture of person-centered care and a culture of safety.

 

  • Demonstrates to peers the correct methods and model behavior needed to address client care needs on an individualized basis.
  • May provide back up in the absence of the DNS.
  • Regular attendance and punctuality is an essential function of the job.
  • Abide by the standards and procedures of the Mennonite Village Compliance and Privacy Program and Values and Expectations Statement.
  • Abides by the HIPAA policies and reports to a supervisor, breaches (or violations) of our HIPAA policies.
  • Immediately reports identified hazards or hazardous conditions to a supervisor.
  • Immediately reports adverse events and near misses to a supervisor.
  • Attends mandatory training on HIPPA and compliance annually.

QUALIFICATIONS:

  • Supportive of the Mennonite Village Philosophy.
  • A current, unencumbered license as Registered Nurse in the State of Oregon.
  • Ability to work with and communicate effectively with the elderly.
  • Capable of transferring residents, with assistance, according to care plan, or as necessary.
  • Ability to safely move frequently during work day, including carrying, lifting, standing, sitting, bending, reaching, and kneeling. Ability to push and/or pull a wheelchair.
  • Ability to be firm and empathetic to staff. Ability to remain objective and treat all staff equitably.
  • Visual and auditory acuity within normal limits.
  • Ability to communicate by phone.
  • Ability to work under time pressure and stress.
  • Must be flexible and responsive to change on short notice.
  • Sufficient English and Language skills to read care plans and communicate effectively with other staff. Must be able to understand and respond properly to resident’s needs.
  • Ability to accurately document activities of daily living.
  • Basic computer knowledge.
