Full Time Visiting Physical Therapist

Carelink Home Health, and its affiliates, deliver compassionate, high-quality care to patients and clients in their homes or places of residence, including Assisted and Independent Living Facilities. Our skilled staff members focus on each unique patient to deliver the appropriate care and emotional support to our patients, their families, and the staff at the facilities they may live in.

 

Coverage Area:

Fulltime | DuPage, Kane, Lake, McHenry, and South Western Cook are the areas we cover

PT - Overview:  

  • Assessing and re-evaluating the client’s condition and needs as required
  • Provide patient cares according to client’s care plan and needs
  • Teach the client and/or family members about cares and support skills
  • Communicate regularly with RN Case Managers, Supervisors, and the Care Team
  • Accurately and thoroughly document patient care observations, interventions and evaluation on the day services are rendered
  • Participate in Performance Improvement activities as assigned. Communicate information that enable collection and root cause analysis of data to identify opportunities for improvement
  • Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of lifeW

 

 PT Qualifications:

·         Licensed as a Physical Therapist in the state of Illinois

·         Valid Driver's License and reliable vehicle with current insurance

·         Prior Physical Therapy experience in a Home Health, Hospital, Rehab, or ALF setting is preferred

·         Experience with Electronic Health Records (currently using Kinnser software)

       Comfortable with technology and must have a smart phone or mobile device

·         Current CPR certification

·         Strong communication and interpersonal skills

·         Great with customer service
