Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay. Our goal is to treat the entire patient while improving their overall wellness and quality of life, in addition to providing free pregnancy tests, HIV/AIDS screening. Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to 10 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. Job Description

The Community Health Worker (CHW) II (Special Projects) is responsible for outreach, education, and preventive health screenings in neighborhoods located near a Legacy clinic to increase awareness of disease prevention and health promotion and to link individuals to medical, dental, behavioral, and other Legacy health services. In addition, the CHWII conducts special assignments in community health that are focused on a unique population in need, a specific content area, and/or implementation of an evidence-based program. Special Project assignments are made as needs are identified.

Essential Functions

  • Provide individual and/or group-level health education and promotion activities in neighborhoods, door-to-door, local businesses, agencies, churches, park, recreation facilities, schools, and more.
  • Develop and conduct presentations to groups of community members at local community sites.
  • Conduct health screenings in both community and clinic settings including (but not limited to) HIV, Syphilis, blood glucose, blood pressure, and pregnancy.
  • Provide intensive in-home counseling in self-management of pregnancy and postpartum care, chronic illnesses, and medication adherence.
  • Develop, secure, and maintain any equipment or instructional materials needed.
  • Develop, secure, and maintain promotional materials (flyers, brochures, calendars, social media, etc.).
  • Promote Legacy service lines and link community members to Legacy as a medical home.
  • Educate about the Legacy Eligibility process and make Eligibility (and other) appointments.
  • Make community referrals to other healthcare services to assist in meeting the needs of clients.
  • Document all community education activities.
  • Under direction of leadership, develop and implement procedures for effective community outreach and health education and evaluate effectiveness of class/counseling content.
  • Utilize resources as directed in order to meet grant expectations and goals.
  • Attend training workshops and meetings as required.

In addition to the Essential Functions listed above, the CHW II (Special Projects) will:

  • Plan, implement, and evaluate programmatic strategies with fidelity related to the assigned project. Strategies include (but are not limited to): community health assessments, outreach and testing campaigns, social networking strategy, home and community-based patient navigation, evidence-based programs, curriculum implementation, and more.
  • Establish and maintain relationships that support regional activities and linkage to care.
  • Plan and implement special events such as community forums.
  • Monitor grant/contract requirements including budgets and reporting, as applicable.
  • Under direction of leadership, develop and implement procedures for effective activities.
  • Participate in the Quality Improvement Program and other performance improvement efforts as directed.
  • Supervise support staff as directed.
  • Other duties as assigned.

Education & Training Requirements

  • Two (2) years in good standing as a CHW at Legacy or in an equivalent role.
  • Holds all certifications required to perform the duties of a CHW in the specific project area or has the ability to obtain certifications within a specified timeframe upon hire. This includes becoming a Texas Certified Community Health Worker (CHW).

Work Experience

Required

  1. Experience providing direct education to people of diverse cultures and backgrounds.
  2. Demonstrated understanding of cultural competence with the focus population.
  3. Experience with public speaking.
  4. Experience with computers and computer programs.
  5. Ability to provide community health screenings within 6 months if/as required per assignment.
  6. Ability to demonstrate core competencies within 3 months of hire.
  7. Demonstrated ability to independently plan, implement, and evaluate educational initiatives.

Preferred

  1. Direct or lived experience with the focus population of a special project as identified.
  2. Bilingual English/Spanish.
  3. Phlebotomy trained.
  4. Experience working in an electronic health record system.
