POSITION SUMMARY:
Manages the day-to-day health and wellness operations of the community to ensure resident’s needs are met.
ESSENTIAL DUTIES:
1. Assists in the community staffing of Care Partners, Med Techs and nursing staff.
2. Provides training and education to associates on an on-going basis to include classroom in-services and if required, situation specific training.
3. Assists in the maintenance of resident charts and review of documentation performed by care staff.
4. As allowed by the Nurse Practice Act, assists in assessing the health, functional, and psychosocial status of residents. Initiates individualized Personal Service Plans and proactively manages care and services for each resident. Evaluates the effectiveness and maximizes the resident's opportunity to remain in their environment.
5. Assists in the achievement of occupancy goals by monitoring the health, safety and well-being of residents, and identifying risks for move-outs.
6. Oversees and manages the consistency of medication training, pharmacy management and medication supervision and/or administration within the community.
7. Conducts periodic Care Partner medication skills, inventory checks and periodic medication audits.
8. Monitors and participates in Quality Improvement and Risk Management programs with monthly QA meetings, care meetings and family conferences.
9. Communicates in residents’ health and well-being, personal needs, risk management issues, resident issues, family or outside healthcare provider concerns.
10. Participates in community events, including family and social functions
11. Other duties as assigned by supervisor.
JOB QUALIFICATIONS:
1. Must be a LVN/LPN or Registered Nurse with minimum five (5) years’ experience, preferably in Assisted Living
2. Demonstrated knowledge of nursing practices, techniques and methods applied to health and wellness resident care; federal and state and local regulations
3. Demonstrated knowledge of good assessment skills
4. Ability to handle multiple priorities
5. Possess written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care
6. Competent in organizational, time management skills
7. Demonstrates good judgment, problem solving and decision-making skills
8. Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications
