Corvallis Caring Place (CCP), a not-for-profit assisted living facility located in Oregon’s beautiful Willamette Valley, provides compassionate, quality care for seniors who need assistance with activities of daily living. CCP is a 42-apartment facility that offers affordable studio and one-bedroom apartments to seniors.  We are currently seeking a compassionate Registered Nurse (RN) for the role of Health Services Director.


The full-time RN position oversees the health care services provided for residents. Oversees and consults on hiring, training, supervising and evaluating performance of health services staff in the provision of care to residents. Assesses and directs nursing management of residents at move-in and upon change of condition. Follows the Oregon Administrative Rules (OARs) as well as the Policies and Procedures of CCP. 

 

 Essential Functions:

  1. Assists the Administrator in the pre-screening process and evaluation of persons with housing and medical service needs appropriate for the assisted living setting. Leads in the decisions about health services staff ability to provide quality services for residents with high levels of acuity. Oversees resident discharges.
  2. Provides nursing expertise by regularly evaluating residents’ health-related needs and changes of health condition. Assists in writing service plans that describe current resident needs, communicates information to direct care staff and families, and guides staff in direct care delivery. As needed, communicates with physicians about resident illness/health and ensures that physician orders are followed.
  3. Provides on-going training and guidance for Medication Technicians. Assigns tasks that are appropriate for assisted living settings and based upon staff qualifications and willingness. Oversees accuracy in ordering and administering medications and treatments.
  4. Supervises the Resident Care Coordinator and Medication Technicians, including hiring, training, coaching/disciplining health services staff. Performs performance reviews in a timely manner and as per CCP policy and procedure. Maintains appropriate personnel documentation related to above responsibility areas.
  5. Ensures appropriate documentation of health services provided by all nursing staff.
  6. Promotes teamwork with all health services staff to provide excellent resident services.
  7. Ensures compliance with Oregon Assisted Living regulations as they pertain to medical and health services functions.
  8. Interacts with families, physician office staff, pharmacists, home-health practitioners, Hospice staff, and others to ensure resident needs are met with effective management of services; promotes good facility public relations during interactions.
  9. Oversees the investigations of resident or staff occurrences and identifies safety hazards. Evaluates safety concerns appropriately and promotes safety practices in providing health services.
  10. Performs other duties as determined by the Administrator.

  • Must possess current, unencumbered nursing license in the state of Oregon
  • Must be knowledgeable in the development of:
  1. Service Plans
  2. Medications and safe med-pass practices
  3. Infection control practices and procedures
  4. Fire and life safety
  5. Resident Rights
  6. Behavior management and documentation practices
  • Must have the ability to plan, organize, and direct care
  • Ability to write policies and procedures related to health services practices
  • Must be a team player and willing to participate in direct caregiving activities
  • Must be able to relate to residents, staff, and visitors in a courteous, diplomatic manner in all circumstances
  • Ability to work with and communicate effectively with the aging and persons with dementia
