JOB SUMMARY
Under the direction of Assistant Program Manager, this position provides support services for consumers who have intellectual or developmental disabilities; supervision, direction, and training to Direct Support Professionals (DSPs)); and related duties as assigned. This person is responsible for upholding the waiver mission and the standards of CARF utilizing the principles of self-determination and quality of life while maintaining the recipient in the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Participates with Program Director and Assistant Program Director in developing, implementing, and maintaining effective consumer services in compliance with Volunteers of America Greater New Orleans and funding source licensing requirements.
2. Assists consumers with developing skills to fully participate in community life and accessing service to meet their needs in their home or as needed in the community.
3. Functions as primary liaison to governmental agencies and social service agencies that provide financial assistance and services to the consumer.
4. Completes and submits required documentation within approved timelines.
5. Provides direction to assigned Personal Care Attendants by providing training, orientation, counseling, evaluation, and discipline according to Supported Living Services Program and Volunteers of America Greater New Orleans, Inc. policies and procedures.
6. Develops Plan of Care to meet the needs of the consumer.
7. Provides twenty-four-hour on-call coverage to consumers.
8. Serves as staff backup when a PCA calls out and no other PCA is available. PCA job description must be reviewed and PCA/Facilitator Information form must be completed.
9. Performs related duties as assigned within the job manual and as assigned by the Assistant Program Manager and Program Manager.

ADDITIONAL DUTIES
• Assists consumers with financial management
• Responsible for medication administration records and communication with RN.
• Scheduling staff to work in consumer homes.
SUPERVISORY DUTIES
• Direct Support Staff or Personal Care Attendants
KEY CONTACTS
• Support Coordination Agencies such as Quality Support Coordination, Easter Seals
• Jefferson Parish Human Service Authority, Metropolitan Human Service District and Florida Parishes Human Service Authority
• Other Community Human Service Organization not limited to ARC, STARC, protective services, Families Helping Families, etc.

KNOWLEDGE, SKILLS, AND ABILITIES
This position requires the incumbent to have completed high school and have four years of experience working with individuals who are developmentally disabled. Post high school/college course work strongly preferred. Completion of a four-year degree in a human service discipline may be substituted for experience. Demonstrated proficiency in developing and writing care plans, ability to advocate for consumers, have EXCELLENT written and verbal communication skills, be a self-starter, and have excellent organizational skills. Must be able to communicate in a clear, concise, and professional manner.

SPECIAL REQUIREMENTS
• Current driver’s license issued by the state of residence
• Current Vehicle Inspection decal
• Current vehicle insurance
• Current vehicle registration in the state of residence
• Satisfactory Motor Vehicle Report
• Must be able to pass a criminal history investigation.

PHYSICAL REQUIREMENTS

• Works primarily in an office environment interacting with staff, candidates, and outside vendors. May be subjected to multiple interruptions throughout the workday.
• Must be able to travel to consumers' homes in a personal vehicle and work in other consumer-related locations.
• While performing the duties of this job the employee is frequently required to sit, stand, walk, use hands to finger, handle, or feel and talk or hear, or carry light training materials and/or equipment.
• The employee is occasionally required to lift up to 25 pounds when preparing/transferring training materials and equipment.
• Visual acuity necessary to operate computers, audiovisual and other digital equipment
• Must be able to travel to consumers' homes and other consumer-related locations in personal vehicles.
• Occasional weekend and/or evening hours may be assigned as needed.
