Weekend Visiting Registered Nurse

Looking to stay busy on weekends?  Now is your chance to join an amazing team!  We are looking for a dedicated RN for weekend admissions and overflow routine visits.  Must be open to travel.  Up to 3 Start of Care each day or a combination of routine / start of care visits as needed.  We will coordinate Friday on the anticipated visits.

Job Description

Registered Nurse - Overview:

  • Completes an initial assessment of patient and family to determine home health care needs
  • Assessing and re-evaluating the patient’s condition and needs as required
  • Provide patient care according to patient’s care plan and needs
  • Teach the patient and/or family members about care and support skills
  • Communicate regularly with Clinical Care Coordinator, Interdisciplinary Care Team, and QA
  • Accurately and thoroughly document patient care observations, interventions, and evaluations on the day services are rendered
  • Participate in and share data for Performance Improvement activities to identify opportunities for patient outcome improvement 
  • Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life 

Qualifications:

  • Licensed as a Registered Nurse in the state of Illinois in good standings
  • Valid Driver's License and reliable vehicle with current insurance
  • Demonstrated ability to work autonomously
  • Prior Nursing experience in a Home Health, Hospice, Rehab, or Hospital is required
  • Must complete start of care OASIS timely and accurately
  • Experience with Electronic Medical Records (EMR) software is preferred
  • Comfortable with technology and must have a smart phone or mobile device
  • Current CPR certification
  • Strong communication and interpersonal skills
  • Great customer service

Requirements:

  • Work with scheduler on Friday to coordinate visits for the weekend
  • Most have Medication Reconciliation and SBAR completed by 9:00 am Monday Morning
  • 48 hours for OASIS
  • Update primary care physician Monday Morning on new start of cares
