Our skilled staff members focus on each unique patient to deliver the appropriate care and emotional support to our patients, their families, and the staff at the facilities they may live in.

Job Description

Speech Therapist - Overview:

  • Evaluates client's speech, language, cognitive, and swallowing abilities to create a plan of care
  • Provide patient care according to patient’s care plan and needs
  • Teach the patient and/or family members about care and support skills
  • Communicate regularly with Case Managers, Supervisors, and Interdisciplinary Care Team
  • Accurately and thoroughly document patient care observations, interventions, and evaluations on the day services are rendered
  • Participate in and share data for Performance Improvement activities to identify opportunities for patient outcome improvement
  • Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life 

Qualifications:

  • Licensed as a Speech Language Pathologist in the state of Illinois
  • Valid Driver's License and reliable vehicle with current insurance
  • Demonstrated ability to work autonomously
  • Prior Speech Therapist experience in a Home Health, Rehab, or Hospital with adult or geriatric population
  • Experience with Electronic Medical Records (EMR) software is preferred
  • Comfortable with technology and must have a smart phone or mobile device
  • Current CPR certification
  • Strong communication and interpersonal skills
  • Great customer service
