Oaks Integrated Care is currently seeking a Case Manager to join the team at our Hattie Talley program in Mt. Holly. Hattie Talley partial care program helps adults diagnosed with a developmental disability and mental illness move to a higher level of functioning at home and in the community. The qualified candidate should have the ability to identify strengths & weaknesses and provide advocacy for consumer needs; have the ability to convey respect and gain trust with the population served; be computer literate with a strong knowledge of MS Word and Excel; and have excellent written and verbal skills.

Schedule: 40 hour work week, Monday to Friday 8:30am to 4:30pm; meals shared with individuals served

Responsibilities:

• Provide supportive interventions to clients including linkage to community and ancillary resources

• Assist in development of activities of daily living including increasing independence, coping skills and skill building

• Respecting consumer right to self-determination during the development and continuously review the consumer’s Individualized Recovery Plans (IRP)

• Participate in team meetings with consumer, family, support network and other providers, including residential programs as appropriate

• Maintain records to include progress notes, Individual recovery plans, discharge and transfer summaries and all other records required by agency policies

• Provide support to medical team as well as act as a liaison with clients to maintain medical appointment schedule

Qualifications: 

  • Bachelor's Degree in Human Service plus two (2) years experience in providing human services OR;
  • Associate's Degree plus five (5) years experience in providing human services OR;
  • Possess relevant professional credential such as Certified Psychiatric Rehabilitation Practitioner, BA Degree in Social Work, Certified Rehabilitation Counselor, Certified Alcohol and Drug Counselor, BA Degree in Rehabilitation, or BA of Science in Nursing;
  • Must possess a valid driver’s license in good standing; bilingual a plus.
