The Home Health Case Manager is responsible for ensuring and coordinating quality and safe delivery of home health services to its clients in the Home Health Agency Private Duty department. 

Your Contributions:

  • Performs all client assessments, obtain necessary authorizations and consents and develops and institutes the clients care plan at admission, resumption and re-certification visits.
  • Obtains initial and interim physician's orders and reports on progress and changes in client condition to Physician/Clinical Supervisor of Private Duty.
  • Performs reassessment and case review as appropriate and timely revisions of the individualized care plan.

Your background includes:

  • Graduate of an approved school of nursing
  • Currently Licensed to practice nursing in the State of Florida
  • Previous geriatric or home care background helpful
  • Home Health Agency experience preferred
  • Minimum 2 years supervisor experience preferred
