POSITION GOAL

The Program Coordinator (Child and Adolescence/EPSDT Programs) will provide program oversight for the Child and Adolescent Health program in conjunction with the FCHA Director according to the guidelines designed by the Iowa Department of Public Health. The CAH/EPSDT Coordinator will also build and maintain a partnership with community human service agencies and medical offices.

  

Wage and Schedule

Monday-Friday 8:00 am - 5:00 pm; $22.36-$23.13 per hour

*This position is eligible for a $1200 retention incentive after successfully completing 180 days of employment.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Provide oversight, direction, and support for the CAH Program.
  2. Responsible for completion of MCAH grant activities and action plans, in conjunction with the Project Director’s guidance.
  3. Serves as the contact for receiving referrals from the AEA for ongoing developmental and emotional/behavioral screening for infants and toddlers (0 to 3 years of age) who did not qualify for Early ACCESS
  4. Coordinates implementation of the EPSDT informing and care coordination services according to required guidelines.
  5. Ensures development and annual review of protocols, scripts and written communication for families.
  6. Monitors Medicaid program and policy guidelines and information related to EPSDT services.
  7. Ensures training of EPSDT staff.
  8. Participates in quality assurance reviews of documentation of services.
  9. Perform direct activities of family-centered care coordination to include: (1) identifying what resources the family needs, (2) informing families about resources, (3) assist in an application for health insurance (4) bridging communication gaps, (5) serving as an advocate for the family, (6) arranging transportation, if needed, (7) providing support and assistance as families negotiate the system, and (8) enabling families to become knowledgeable, independent health care consumers
  10. Follow procedures and guidance to assure compliance with the Department of Public Health MCAH grant.
  11. Maintain current data in state data systems.
  12. Follow HACAP procedure for reporting suspected child abuse and neglect.
  13. Manage a flexible work schedule according to the agreed-upon number of hours scheduled to work per week, in conjunction with the supervisor that meets the needs of the company, clients, contractors, and any other appropriate parties. This may include evening and weekend hours.
  14. Assist with monthly informing caseload, as needed.
  15. Attend required meetings and trainings.
  16. Other duties as assigned.

 

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

  1. A minimum of six months experience in health or human services field.
  2. At least one of the following:
    1. Bachelor degree in health or human services degree.
    2. Current license as a registered nurse (RN) with a bachelor’s degree in any field.
    3. Current license as an advanced registered nurse practitioner (ARNP).
    4. Current license as a registered dental hygienist.

 

 CONDITIONS OF EMPLOYMENT

  1. Complete Child Abuse Reporter training within six (6) months of employment and update every three (3) years.
  2. Complete bloodborne pathogen training within ten (10) days of employment and update every year.
  3. Valid driver’s license.
  4. Valid car insurance.
  5. Reliable transportation for on-the-job use.
