PRN Visiting Occupational Therapist

Our skilled staff members focus on each unique patient to deliver the appropriate care and emotional support to our patients, their families, and the staff at the facilities they may live in.

Coverage Areas:  Kane, Kendall, DuPage | Lake, McHenry | Western Cook Counties (depending on your location)

Job Description

Occupational Therapist - Overview:

  • Assessing and re-evaluating the patient’s condition and needs as required
  • Provide patient care according to patient’s care plan and needs
  • Teach the patient and/or family members about care and support skills
  • Communicate regularly with Nurse Case Manager, Supervisors, and Interdisciplinary Care Team
  • Accurately and thoroughly document patient care observations, interventions, and evaluations on the day services are rendered
  • Participate in and share data for Performance Improvement activities to identify opportunities for patient outcome improvement
  • Supervision of Occupational Therapy Assistant 
  • Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life 

Qualifications:

  • Licensed as a Occupational Therapist in the state of Illinois
  • Valid Driver's License and reliable vehicle with current insurance
  • Demonstrated ability to work autonomously
  • Prior Occupational Therapy experience in a Home Health, Rehab, or Hospital setting is preferred
  • Experience with Electronic Medical Records (EMR) software is preferred
  • Comfortable with technology and must have a smart phone or mobile device
  • Current CPR certification
  • Strong communication and interpersonal skills
  • Time management to balance patient care, travel, and documentation
  • Great customer service
