ABOUT THE ROLE
Childhaven has an exciting opportunity to join the Flourish WISe (Wraparound with Intensive Services) Team as our next Wraparound Facilitator. WISe/Wraparound is a team-based planning and service process designed to create better outcomes for trauma-affected children, to build child and family stability and support children’s success in their homes and communities. Members of this team will work closely together to collaboratively develop and implement an individualized plan of care, monitor the efficacy of the plan, and work towards success over time.

The Wraparound Facilitator coordinates and facilitates the strengths-based and family-driven Wraparound with Intensive Services (WISe) process for trauma-impacted children and families. This position is the care coordinator and facilitator of the multidisciplinary Child and Family Team, and is responsible for leading the team through the phases and activities of WISe. The Facilitator contributes knowledge and skills to ensure the team process honors each member’s role, responsibility, and perspective. Work is guided by infant/early childhood intervention models and provided in a team framework, in close collaboration with Childhaven’s Wraparound Parent Partners, Infant/Early Childhood Mental Health Therapists, and Teachers. Emphasis is on partnering actively with families, natural supports, and other community service providers to promote safe, stable, and healthy families. 

Reporting to the WISe Program Manager, this position requires flexible work schedule, including some evening and weekend hours to accommodate family needs. Services are performed in greater Seattle or Auburn/south King County areas.

TYPICAL DUTIES

  • Prepare for and facilitate Child and Family Team (CFT) meetings, including agenda, visual aids, tools, and resources. Schedule times and locations convenient for family and team members.
  • Assist families in identifying their children’s strengths and needs in order to develop their Individualized family vision. Complete weekly check-in calls with families and maintain complete documentation of the child’s case records.
  • Participate in ongoing, job-related professional development opportunities, in-service trainings, and continuing education (e.g., Child Abuse Awareness, HIPAA, antiracism, etc.).
  • Regularly assess own performance for improvement.
  • Guide and serve as central point of communication for the Child and Family Team process.
  • Encourage each team member to identify priority concerns, minimize areas of potential conflict, and follow mandates of child-serving systems, using consensus-building techniques.
  • If individual team members cannot attend a team meeting, arrange participation via phone or teleconference, or gather their input in advance.
  • Identify the strengths and needs of the child and family, provide team members with an overview of Child and Family Team practice, clarify roles and responsibilities.
  • Work with Parent Partner to identify family support or other formal and informal resources to support child and family integration into their community and serve on CFT.
  • Ensure all team members receive updated copies of the Cross-System Care Plan, documentation of progress, CFT notes within 7 days after each meeting.
  • Maintain team focus on progress toward established goals and transition out of WISe.
  • Ensure respect for the input, needs, and feelings of the child and family.
  • Inform the child and family of their rights, obtain consents and releases of information.
  • Acknowledge and celebrate successes and transitions.
  • Provide or arrange crisis response and stabilization services. Complete, update, and communicate the Family Safety and Crisis Plan with family and team members.
  • Participate in 24/7/365 on-call Crisis Response. Respond to client crises during assigned weeks.
  • Ensure compliance with all Childhaven policies and procedures. Adhere to all related contracts, Federal, State, and local requirements, and guidelines.
  • Maintain documentation of child’s case records and electronic health record (EHR) according to Childhaven policy, County and State requirements, and Wraparound fidelity guidelines. Perform reviews of documentation when required by WISe Coach or when need arises (self-audit).
  • Maintain productivity standards for direct service as established for the position.
  • Ensure timely collection of statistical information for funding sources and program evaluations.
  • Participate in scheduled supervision, team meetings, and administrative meetings. Utilize reflective supervision to support professional growth and quality client care.
  • Remain current with professional development requirements.
  • Other duties as assigned.

REQUIRED QUALIFICATIONS

  • 2 years of related experience working or volunteering with children and families in the field of social services or early intervention, preferably in community-based services.
  • Knowledge of early childhood development with a demonstrated ability to foster relationships with parents, infants, and toddlers.
  • Knowledge of local and statewide family resources.
  • Knowledge of social justice and racial equity concerns that affect families; experience interacting with diverse populations.
  • Able to effectively communicate; has the skills and abilities to develop and maintain good public relations among clients, volunteers, other personnel, and the public.
  • Able to work effectively with diverse clients and agency staff; be aware of other needs and advocate for safety.
  • Warm and compassionate approach with the ability to establish rapport and provide emotional support to families.
  • Exceptional people and communication skills, both verbal and written.
  • Be in acceptance of a variety of lifestyles, behaviors, and cultural/spiritual differences.
  • Ability and desire to be flexible and responsive
  • A valid driver’s license and a safe driving record with state-required minimum automobile insurance; able to use personal vehicle for the job.
  • Ability to obtain and/or maintain current certifications including Pediatric/Adult CPR, first aid and AED, HIV and blood borne pathogens
  • Background check, drug screen, tuberculosis test, and COVID-19 vaccination required.

DESIRED QUALIFICATIONS

  • Experience in WISe or other Wraparound models.

To achieve this, Childhaven uses a grade and step approach to determine the compensation of each team member based on years of relevant experience.
