POSITION GOAL

To promote HACAP’s Support Services for Veteran Families Program (SSVF) within the community and conduct intake screenings with homeless veterans in the assigned area. This position will work with other community partners in identifying homeless veterans, referring them to eligible services, and working in the community to end veteran homelessness. Position will be based in Dubuque and responsible for outreach in Dubuque, Jackson, Delaware, Buchanan and Black Hawk Counties.

 

Wage and Schedule

Monday-Friday 8:00 am- 5:00 pm (some non-traditional hours may be needed); $24.14 hourly. 

*This position is eligible for a $1200 retention incentive after successfully completing 180 days of employment.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Conduct outreach for homeless veterans in places where the most vulnerable in the community may congregate.
  2. Follow stated outreach policies and procedures as defined by the VA and HACAP.
  3. Refer screened but not enrolled veterans to other local community resources for services.
  4. Explain program expectations and eligibility requirements to applicants.
  5. Work with interested veterans on helping them obtain needed eligibility documentation.
  6. Use a Housing First approach with veterans to engage them in the program.
  7. Represent HACAP and the program at outside community events and in a proper manner
  8. Submit outreach activities and applications for program to the SSVF Compliance Coordinator
  9. Case manage a 50% case load of enrolled veterans based on community need and program capacity.
  10. Attend staff and case conferencing meetings within the program and with other community partners as deemed appropriate.
  11. Other duties as assigned by management staff

QUALIFICATIONS

TARGETED EDUCATION and/or EXPERIENCE

Bachelor degree in social service field of study and 2 years of experience

OR

Seven years related experience and/or training.

OR

Equivalent combination of education and/or experience.

 

Skills and/or Abilities 

  1. Ability to communicate effectively (oral and written)
  2. Have experience working with individuals with high barriers
  3. Familiarization with the mental health, VA, and homeless systems in the community
  4. Knowledge of Housing First practices
