JOB SUMMARY

This position is responsible for providing case management to veteran families. The Case Manager will perform in a manner that successfully ensures that quality outcomes are achieved, prepare documentation, and perform related duties as assigned.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Connect Veterans with benefits and supports.
  2. Provide support and direct assistance to Veterans based on an individualized service plan.
  3. Assist/train Veterans in daily life skills. Transportation, communication, and self-advocacy.
  4. Maintain assigned program records.
  5. Monitor and track progress of Veterans with housing needs.
  6. Develop, implement, monitor, evaluate, and modify person–centered housing plans.
  7. Advocate for needed services including entitlements and benefits for persons’ served.
  8. Ensure obligation of elements to tenancy have been met by tracking Temporary Financial Assistance (TFA).
  9. Ensuring referrals and due diligence is provided and documented.
  10. Problem-solve conflicts and assist other team members as needed.
  11. Prepare and maintain assigned program records and documentation.
  12. Complete intakes for veteran families in need of Supportive Services for Veteran Families (SSVF) assistance
  13. Data entry and compliance of SSVF grant
  14. Provide transportation as needed to families in need of services.
  15. Conduct outreach in bayou parishes of Southeast Louisiana such as Assumption, Lafourche, St. Charles, St. James, St. John, St. Mary and Terrebonne Parishes.
  16. Performs other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES

 

This position requires knowledge, skills and abilities equivalent to:

  • Bachelor’s degree level and one year case management experience working with individuals that are rapid-rehousing and homeless prevention.
  • Have a demonstrated willingness to be responsive to the needs of the target population, excellent verbal and written communication skills and the ability to function as part of a team.
  • All candidates must be able to handle a flexible work environment and have strong decision making skills.

 

WORKING CONDITIONS

Normal office environment

This position may require incumbent to engage in overnight travel, as well as, local travel appropriate to the performance of duties and responsibilities. As such, incumbent must have:

  • Current driver’s license issued by state of residence
  • Current Vehicle Inspection decal
  • Current vehicle insurance
  • Current vehicle registration in state of residence
  • Satisfactory Motor Vehicle Report 

PHYSICAL REQUIREMENTS

Work is primarily performed in a normal office environment while seated at a desk operating a personal computer keyboard, calculator, phone and other office equipment.
