Position:  Case Manager DBMD (Deaf, Blind, and Multiple Disabilities)

Location:  Euless, TX

Pay:  $48,000 per year

Primary Responsibilities:

  • Coordinates the deliver of Waiver services to the VOATX participants.
  • Organizes and directs services to ensure compliance with applicable federal law, state certification principles for participation in the Deaf, Blind, and Multiple Disabilities program, and VOATX policies.
  • Ensures that supports are delivered by qualified individuals.
  • Ensures that all staff receives adequate and timely training, re-training, and evaluation.
  • Responsible for the development of each participant's Implementation Plan related to the Individual Program Plan (IPP).
  • Informs individual and/or his/her legally authorized representative about recipient's IPP, any agreed-upon changes in IPP prior to implementing the changes, and be available to answer their questions about the IPP.
  • Ensures that documentation of all supports delivered to participants is accurate and timely in accordance with VOATX policies and the applicable DBMD billing guidelines.
  • Ensures appropriate Level of Need assignments an initiates requests for changes to DADS.
  • Ensures that services are delivered timely and in accordance with participants' plans of care and progress toward identified outcomes are documented and monitored.
  • Ensures all state required documentation including IDIRCs, IPPs, ICAPs, quarterly reviews, etc. are completed by required deadlines to ensure continuity of care.
  • Records and maintains documents in a timely manner; completes staffing reports, IPCs, IPPs, ICAPs, training documentation, case notes, and other required documentation.
  • Reviews at least monthly, all DBMD participants' utilization and adds/edits/deletes services as needed and ensures services have been delivered timely and all billing has been collected.
  • Supervises and coordinates transportation  as needed.
  • Coordinates participants' needs in scheduling and arrangements to facilitate participants' programs and/or care plan needs.
  • Ensures the health and welfare of all participants and coordinates medical needs with Nursing.  

Qualifications:

  • Bachelor's degree in social sciences or related field plus one year experience with intellectual and developmental disabilities OR high school diploma/GED plus five years direct experience with intellectual and developmental disabilities OR the equivalent combination of education and experience.
  • Social work certification and supervisory experience are highly preferred.
  • Competency in ASL preferred.
  • Valid Texas Driver License, clear MVR, and ability to be insured.
