Position: Clinical Director 

Department: Care Management      

Reports to: Chief Clinical Officer

FLSA Status: Exempt - Full Time (40 hours per week)

 

Position Summary:

Provide leadership and ensure quality outcomes of care managers for participants. Oversee the monitoring and documentation of compliance with federal, state, and local governmental and agency requirements.  

Supervisory Responsibilities:

Provides supervisory responsibility in accordance with CareWell Services policies and procedures. Oversee the Clinical Manager.

Duties/Responsibilities:

  • Development and oversight of services provided to participants through collaborative team approach. 

  • Oversee development of Person-Centered Service Plans, level of care determination, linking to appropriate resources, service delivery and service follow-up to assure quality services.
  • Provide leadership to the care management team to encourage evidence-based practices.
  • Monitor training and support to the care management team.
  • Collaborate with staff and service providers by guiding staff to think creatively, resolve conflict and prioritize participant’s needs, goals, and interests.
  • Perform Clinical Quality initiatives in collaboration with the Quality team to ensure compliance with CQAR, AQAR standards, and Quality Indicator Performance, develop and implement training for care management as necessary.
  • Collaborate with Leadership Team to solve operational problems and to improve services.
  • Conduct audits on clinical documentation and care plans.
  • Monitor clinical outcomes. (hospitalization, critical incidents)
  • Supports achievement of agency’s mission and vision and other duties as assigned.

Education:

  • Must possess a current Michigan RN or Social Work license.
  • Must possess a bachelor’s degree.
  • Master’s degree preferred.

Experience:

  • At least five years of human services experience working with disabled and aging (60+) population.
  • Knowledge of Medicaid eligibly requirements.
  • Supervisory experience.
  • Knowledge of Aging & Adult Services and MI Choice Waiver.

License and /or Certification:

  • Possess a current Michigan RN or Social Work license.
  • Current CPR Certification preferred (ACLS preferred)
