The Oaks at Flower Mound is seeking a compassionate and experienced Charge Nurse (LVN/LPN) to join our Assisted Living and Memory Care team. This leadership role combines hands-on resident care with clinical oversight and support of the caregiving team.

Responsibilities

  • Provide direct resident care services and oversee clinical operations during assigned shifts
  • Administer medications in accordance with physician orders, state regulations, and community policies
  • Complete required documentation, reporting, and follow-up activities accurately and timely
  • Conduct resident assessments as required by state regulations and community policy
  • Observe residents for changes in condition and report concerns promptly
  • Maintain and update individualized care plans
  • Document resident care, observations, communications, and clinical procedures by the end of each shift
  • Support, mentor, and guide caregivers to ensure quality resident care
  • Assist with staff training and daily leadership of the care team
  • Promote compliance with community policies and regulatory requirements
  • Foster a positive, resident-focused team environment
  • Perform other duties as assigned

Qualifications

  • Current Texas LVN or LPN license required
  • Current CPR certification required
  • High School Diploma or GED required
  • Minimum one (1) year of nursing experience required
  • Assisted Living, Memory Care, Long-Term Care, or Skilled Nursing experience preferred
  • Previous Charge Nurse or leadership experience preferred
  • Strong communication, organizational, and leadership skills
  • Ability to work independently and make sound clinical decisions
  • Ability to stand, walk, bend, lift up to 25 pounds unassisted, and assist residents with mobility
