Position Summary
The Licensed Practical Nurse (LPN) is responsible for providing direct resident care, administering medications, conducting assessments, and supporting the overall health and well-being of residents. This position plays a vital role in delivering exceptional care while ensuring compliance with all applicable regulations and community policies.
Responsibilities
- Provide direct resident care services while following individualized plans of care.
- Administer medications in accordance with physician orders, state regulations, and community policies.
- Complete all required documentation, reporting, and follow-up related to medication administration and resident care.
- Perform resident assessments as required by state regulations and community policies under the direction of the Director of Assisted Living.
- Observe residents for changes in condition and promptly report concerns to the appropriate supervisor.
- Document resident care, observations, communications, clinical procedures, and daily assistance provided by the end of each shift.
- Maintain and update individualized care plans for residents.
- Assist with resident and family communication regarding care needs and changes in condition.
- Support and collaborate with caregivers and other team members to ensure quality resident care.
- Perform other duties as assigned.
Leadership & Supervision
This position works under the direction of the Director of Assisted Living and/or Director of Memory Care and is expected to exercise sound clinical judgment and work independently. The LPN may assist with the supervision, training, and direction of caregiving staff and participate in performance evaluations and coaching as needed.
Qualifications
Required
- Active Licensed Practical Nurse (LPN) license in the State of Colorado.
- Current CPR certification.
- High school diploma or GED.
- Minimum of one (1) year of nursing experience preferred.
- Strong communication, documentation, and customer service skills.
- Ability to work independently while collaborating effectively with the care team.
Physical Requirements
- Ability to sit, stand, walk, bend, stoop, kneel, crouch, reach, and climb stairs throughout the workday.
- Ability to lift up to 25 pounds independently.
- Ability to assist with transferring, repositioning, pushing, and pulling residents using proper equipment and assistance when required.
